Ability to add a "more information" button to patient field section of intake form

In Netherlands we ask for social security number (BSN) on intake form. Some people are a little concerned giving it over an online form, and then don’t fill the intake form. If we could add a little question mark more information button like we can do in other parts of the intake form, we could make a little explanation then they would still fill the form
