Ability to create a form to update patient details

We periodically update our database with latest details of clients. It would be good to be able to do this with a form. Preferably the ability to show what we have on file and they can amend where wrong, and fill in any blanks. Or it could be activated from the check in portal, as a function that we could turn on and off periodically.
