Add 'default duration' as an option into the practitioner duration override for an appointment type

So that if you update the default duration of an appointment type, all practitioners set to the default will be updated too (and only practitioners who are set to a different duration as an override will remain the same). Currently when you change the default duration of an appt type you also have to change each practitioner's duration override manually
