Add option to prevent auto-capitalisation of patient name

Currently when adding a patient, the first letter of all surnames is automatically capitalised. This isn't true of many Dutch, Belgium, Scandinavian surnames e.g. van Winden, de Wit, van der Velden. Can we add an option to override this.
