Add Stopping Care reason from patient file

Ability to add a stopping care reason to their file from their profile, not just from the recall. Sometimes you assign a recall, get in contact with that person, but you don't get the reason people are stopping care for a few days/weeks and then you have to go into the future recalls to find it and then assign the reason - basically, it never gets done! It's a very cool feature and I'd like to report on it monthly to see where we can improve. Another reason for using it properly is that it would be great to know which of those people it is worth reaching out to again in the future. I.e. if they stopped care because they felt better then it might be worth keeping in contact with that person more regularly than say someone who has moved to another country.
