Appointments Schedules for Patients

We would like to be able to print out a patient schedule for a patient when the are in the office (instead of writing out appointment card). It would be good if our office info could be on it for example address and phone number. We would also like to be able to print them out on a smaller sheet. So not giving a A4 sheet each time
