Client /Pt. facing care plan tracking, let patient see their target

With clients getting front desk independence with an app for rescheduling and making their bookings, it's essential that they can see their care plan and associated tracking to establish where they are in their care journey. As well as a countdown to review points - this can give a sense of accomplishment and guide the distance to check-in points.

Indeed, with this data already being present with PH software, It seems like a missed trick not to have this available in the care plan data in app or be included in emails via connect
