Consent button on Sign -In

Adding a 'consent to treatment' button on Sign in, for "conditions already diagnosed or treated previously by the Chiropractor". A notice of "consent given to treat previously diagnosed or treated conditions" can then automatically pop up on the subjective notes for the day? Would this work for consent purposes?
