Custom Form Submission

Sometimes patients make a call to book in for an appointment on behalf of someone else as a gift etc, if we sent across the form to be submitted pre appointment it might not be sent to the person receiving the gift as they don’t want them to know yet, because the forms auto fill with data, it would be a good idea if rather than the forms opening up with all the details prefilled if to be able to access the form they would need to enter a password or something otherwise whoever’s number was given on the day of appointment booking, they would be able to access the other persons details easily.
