Customisable cancellation charge when cancelling

A settings option to charge custom automatic missed appointment/cancellation fees depending on the appointment type. This way the different appointments with different prices are accounted for. This could be altered in the settings for each appointment type. In addition it would be good to be able to charge 50% of an appointment fee for the first time then 100% of the fee on subsequent cancellations if this could be an option.
