Deactivate links to forms once submitted

Once a form has been submitted through the link sent out, the link to the form should then automatically be deactivated. I worry that patients have this active link where they can potentially keep submitting the form.

Also, is there any way where once the form has been submitted, for it to not have the timestamp on the submitted form?
