Don't share account admin contact details on patient documents

Currently the downloaded notes, which are then shared with patients, shows the account administrator’s email/contact details in the header. It would make more sense for this to default to the Notifications & Reply To Email set in the general settings of the account as this is the one already used by patients.

There is no reason why patients should have the administrator’s details shared with them, as often they may have nothing to do with patients or their communication.
