Filter by practitioner/appointment type when creating a statement and add additional info e.g. HCPC numbers etc onto statements

Filtering by additional fields so that statements only show specific appointment types/practitioners. Also info such as pracitioner's GCC/HCPC numbers etc. to be made visible on statements, as they are on individual invoices.
