Forms to Live System

I think I have already mentioned this before, but as we have just started using custom forms for client enrolment - its slightly frustrating that some of the info collected cant be put straight into the live system rather than just in the notes. Some fields already exist, others dont but would be handy to have in the "Patient Record" - our CA's have just listed.... Marital Status, Occupation, GP info and if the client has Children (YES/NO)
