Inerasable Journal / Permanent Log entries

Our practice would like the inclusion of a feature where the stickynotes area is, specifically a Journal-entry system, with the layout much like a notebook where the doctor/specialist will be able to add permanent and undeletable / unerasable lines of text and information for every visit compiled on a single page (much like a notebook). A strike-through & underline feature would be much appreciated as well. If this feature could be added under the profile (image) of the patient overview page, is our preference.
