Patients to select date of birth instead of name when booking online

Create the option for patients to specify themselves by selecting their date of birth rather than name when online booking. Occasionally if a patient is saved on the system as a slightly different name (e.g. Mike rather than Michael) they cannot find themselves when attempting to book online.
