patients writing down their own subjective of the SOAP section

I am looking into streamlining our proces. Right now pts come in the adjusting room, i have to ask how they are doing, write that down which takes time and they focus on their complaint. Not a positive suggestion to start your adjusting too! I would like: pt comes in, checks himself in (in waitingroom or adjusting room). When they do, the next question is: anything i need to know, any special things happend and what is going better. They fill that in themselves (that goes straight into the subjective part of the SOAP note. They finish writing, click finish and new screen comes up with something like:, find a table when you are ready. Lie down, relax, breath and visualise your best self. Our something like that (maybe we could even customize that and change that every week ourselves as to give them a different exerice every week.
