Pinned notes and/or additional information fields

It would be really useful to be able to pin a practitioner form to the top of the patient's history, so that it was always visible. Alternatively in the side bar of the practitioner notes where you have "Chief Complaint" etc. the ability to add additional fields would solve the same problem. We routinely need to list patient goals, aspirations, expectations, contraindications etc. so having them continuously visible would be great. We currently list contraindications in the flags boxes but technically they aren't red or yellow flags.
